Elementary, Middle School registration set  Share
Registration for Kindergarten-6th grade will be held at the Neoga Middle School from 8 a.m.-8 p.m. on Wednesday, Aug. 5. Kindergarten students must have a certified copy of their birth certificate and a copy of their social security card on file. At registration parents will receive information for the coming school year including school calendar, busing information, student handbook, a supply list and other important items. Book fees for grades Pre-K through 6th grade will be payable at the time of registration. The 2009-2010 school year fees are $40 per student. Physicals for Kindergarten students will be held from 4-6 p.m. Aug.5 at Neoga Middle School. Parents must fill out the necessary information and sign the physical. Physical forms are available in the school office or after Aug. 1, will be available on the school website www.neoga.k12.il.us.
Free physicals will be provided Aug. 5 from 5-6:30 p.m. and on Aug. 6 from 1-3 p.m. at the High School. Each physical must be signed by the parent and filled out completely before seeing the physician. Sports physicals should be completed before the beginning of the sport, and no sooner than 6 months prior to the start of the sport. The following students will require a physical to be on file with the nurse before beginning school: In addition: Kindergarten students must also have a dental exam, eye exam by an eye doctor, lead screen, diabetes screen and current immunizations. Second graders need a dental exam. Sixth graders should get updated immunizations and a dental exam. Forms for physicals, eye exams, and dental exams are available at the school office.
